Zusammenfassung

Die neonatale Alloimmunthrombozytopenie ist die wahrscheinlichste Ursache schwerer Thrombozytopenien (< 20 000/µl) bei ansonsten gesunden Reifgeborenen. Sie wird häufig erst im Rahmen einer neonatalen Hämorrhagie festgestellt und kann bei verspäteter Diagnose und Therapie schwerwiegende Konsequenzen für das betroffene Neugeborene haben.

Fallbericht eines eutrophen männlichen Reifgeborenen mit ausgeprägter postnataler Blutungsneigung und hämorrhagischer Infarzierung der rechten Hemisphäre im Rahmen einer Alloimmunthrombozytopenie.

Bei unauffälligem plasmatischen Gerinnungsstatus und ausgeprägter Thrombozytopenie wurden im mütterlichen Serum thrombozytenspezifische Alloantikörper der Spezifität Anti-HPA-1a nachgewiesen. Nach adäquater Substitution mit HPA-1a-negativen Thrombozyten kam es zu einer Stabilisierung der Thrombozytenzahl. Als typische schwere Komplikation der erhöhten Blutungsneigung fand sich eine mit großer Wahrscheinlichkeit schon intrauterin aufgetretene intrakranielle Hämorrhagie mit Infarzierung der rechten Hemisphäre.

Die Feststellung einer ausgeprägten Thrombozytopenie als Ursache der Hämorrhagie erlaubte den raschen Beginn einer Substitutionstherapie mit Thrombozytenkonzentraten. Die Diagnose einer Alloimmunthrombozytopenie sollte im Falle einer erneuten Schwangerschaft zur Betreuung in einem spezialisierten Zentrum mit der Möglichkeit zur Bestimmung der fetalen Thrombozytenzahlen und wenn nötig zur intrauterinen Thrombozytensubstitution führen.

Der vorgestellte Fall zeigt exemplarisch die klinische Symptomatik und die Therapie der neonatalen Alloimmunthrombozytopenie. Hervorzuheben ist, dass bei jedem Neugeborenen mit den klinischen Zeichen einer erhöhten Blutungsneigung an die Diagnose einer Alloimmunthrombozytopenie gedacht werden sollte. Dabei wird die Thrombozytopenie entsprechend ihrem Schweregrad im Rahmen der Akutversorgung des Patienten behandelt. Die Diagnose sollte aber in jedem Fall dazu führen, eine erneute Schwangerschaft in einem perinatalmedizinischen Zentrum zu betreuen.

Abstract

Alloimmune thrombocytopenia is the most common cause of severe thrombocytopenia (< 20 000/µl) in otherwise healthy neonates. The diagnosis is usually made in the diagnostic workup of neonatal hemorrhage. If diagnosis and therapy are delayed the consequences for the affected child can be deleterious.

Case report of a full-term neonate with the clinical symptoms of severe hemorrhage and hemorrhagic infarction of the right hemisphere due to neonatal alloimmune thrombocytopenia.

While the plasmatic coagulation parameters were unaffected, we detected severe neonatal thrombocytopenia and anemia and identified alloimmune antibodies against the child’s thrombocytes in the mother’s serum. The antibodies were specific against the platelet antigen HPA-1a. Transfusion of HPA-1a-negative thrombocytes stabilized the platelet count. Anemia was treated by erythrocyte transfusion. Unfortunately, the patient exhibited, most likely intrauterine, intracranial hemorrhage and infarction of the right hemisphere, the most dreaded complication of neonatal alloimmune thrombocytopenia.

The identification of severe thrombocytopenia causing the hemorrhage allowed us to start substituting thrombocytes without any delay. The previous diagnosis of alloimmune thrombocytopenia should lead to monitoring a subsequent pregnancy in a specialized unit allowing fetal blood sampling and intrauterine thrombocyte substitution if necessary.

This case report exemplifies the symptoms and treatment of neonatal alloimmune thrombocytopenia. The differential diagnosis for a neonate showing clinical signs of hemorrhage should include alloimmune thrombocytopenia which can then be treated adequately. The diagnosis should also lead to careful monitoring of subsequent pregnancies.
