Zusammenfassung

Die Begleitverletzung urogenitaler Organe ist bei politraumatisierten Patienten, insbesondere im Rahmen komplexer Beckentraumen, häufig. Eine optimale Versorgung sowohl der parenchymatösen, knöchernen und der Weichteilverletzungen kann nur im Rahmen eines interdisziptinären Managements erfolgen, in welches der Urologe bei entsprechenden Verletzungsmustern von Beginn an eingebunden werden sollte. Nur so kann das Auftreten von gravierenden Nebenwirkungen bei nicht rechtzeitig erkannten und behandelten Verletzungen von urogenitalen Organen verhindert werden: Dabei sollten von urologischer Seite therapeutische Maßnahmen nur unter Berücksichtigung des Allgemeinzustandes und der durch Begleitverletzungen determinierten Prognose des Patienten ergriffen werden. Aus diagnostischer Sicht sollten bei Makrohämaturien im Spontanurin oder Blutaustritten aus dem Meatus urethrae externus, soweit es die Vitalfunktion und das Verletzungsmuster des Patienten zulassen, mittels retrograder Urethrographie Verletzungen der Urethra abgeklärt werden. Nach Ausschluss der Urethraverletzung indiziert eine Makrohämaturie als Leitsymptom der Blasenruptur eine weitere entsprechende Diagnostik mittels CT oder Zystogramm. Therapeutisch werden Harnblasenrupturen in den meisten Fällen primär offen-operativ angegangen, während Urethraverletzungen in der Regel mittels mehrmonatiger suprapubischer Harnableitung und sekundärer offen-operativer Therapie versorgt werden. Weitere Untersuchungen werden den Stellenwert moderner endourologischer und laparoskopischer Techniken bei der Versorgung traumatischer Läsionen des unteren Urogenitaltrakts aufzeigen.

Abstract

In the polytraumatized patient, concomitant lesions of the urogenital tract are often found. To ensure optimal patient care, interdisciplinary management of parenchymatous, bony and soft tissue lacerations must be installed. Given urogenital lesions or an indication for such a lesion, the urologist must be involved early on to avoid serious consequences of inadequately diagnosed or inappropriately treated urogenital lesions. Given an appropriate setting with adequate patient surveillance, most renal injuries can be safely managed by a conservative approach. Only cases with severe renal shattering or injury of the large renal vessels with subsequent severe cardiovascular effects necessitate immediate formal revision. Blood on the penile meatus or gross hematuria in voided urine suggests ureteral rupture and necessitates retrograde urethrography; in case of a normal urethra, bladder ruptures must be excluded by CT scan or cystography. In most cases, urethral ruptures can be safely managed by placement of a suprapubic drain and secondary revision. Bladder ruptures must be formally revised in the majority of cases.
