Interaktionen zwischen Pharmakotherapien und Psychotherapien bei
Depressionen

Abstract

Bei Depressionen scheint eine generelle positive Interaktion zwischen Pharmakotherapien und Psychotherapien zu bestehen. Das gilt vor allem für trizyklische Antidepressiva und selektive Serotonin-Reuptake-Inhibitoren (SSRI). Bei diesen Substanzgruppen fand sich für verschiedene Psychotherapien kein Fall einer negativen Interaktion.
Bei den anderen Antidepressiva ist der Kenntnisstand zu gering für eine fundierte Stellungnahme. Für das frei verkäufliche Johanniskraut, das oft von Patienten in Eigenregie zusätzlich zu einer Psychotherapie eingenommen wird, fand sich keine Interaktionsstudie.
Benzodiazepine und Neuroleptika sind in diesem Zusammenhang bei langfristiger Verabreichung eher negativ zu bewerten. Bei Benzodiazepinen sind Wirkungsverlust und Arzneimittelabhängigkeit zu befürchten, bei Neuroleptika Nebenwirkungen (Motorik, Gewichtszunahme und Sexualität).
Die Kombination Psychotherapie - Pharmakotherapie wirkt sich positiv auf die Compliance der Depressiven aus. Ob dabei die Therapiekombination in einer Hand liegen sollte oder von zwei kooperierenden Therapeuten durchgeführt werden sollte, ist nicht ausreichend untersucht.
