Summary

Epilepsy surgery has been meeting with increasing interest in recent years but is still underutilized in many European countries. Possible reasons for the presently recognized epilepsy surgery „boom” in several countries include the availability of advanced non-invasive diagnostic tools to delineate epileptogenic lesions and epilepsy-related functional deficits. Improved electrophysiological and surgical techniques are, however, equally important. All these improvements translate into better postsurgical outcome figures and into a larger population of difficult-to-treat patients profiting from surgical therapy. Also, one can recognize an important role of epilepsy surgery within the modern neuroscience sphere. A short review and analysis of the present state of the art of epilepsy surgery in Europe is attempted and possible future scenarios for its further development are outlined. The growing need for quality control and multidisciplinary and world-wide collaboration is emphasized.

Epilepsiechirurgie in Europa: Fortschritte im letzten Jahrzehnt und gegenwärtiger Stand

Obwohl die Epilepsiechirurgie zunimmt, ist sie - gemessen am Bedarf - auch in manchen europäischen Ländern immer noch untervertreten. Mögliche Gründe für den teilweise in einigen Ländern festgestellten „boom” sind die nunmehr vorhandenen modernen nichtinvasiven diagnostischen Untersuchungsmethoden um epileptogene Läsionen und epilepsieassoziierte funktionelle Defizite aufzuspüren. Verbesserte elektrophysiologische und chirurgische Techniken sind gleichermaßen bedeutend. Alle diese Verbesserungen resultieren in besseren postoperativen Ergebnissen und in einer größeren Zahl von Patienten mit schwierigeren Epilepsien, die heute von einer operativen Behandlung profitieren. Auch kann man mit Genugtuung feststellen, dass die Epilepsiechirurgie heute eine wichtige Rolle im Bereich der modernen Neurowissenschaften einnimmt. In diesem Artikel wird ein kurzer Überblick und eine „state of the art”-Analyse der gegenwärtigen Situation der Epilepsiechirurgie in Europa gegeben. Zukünftige mögliche Szena-rios werden aufgezeigt. Die Notwendigkeit für qualitätssichernde Maßnahmen und für multidisziplinäre und weltweite Zusammenarbeit wird hervorgehoben.
