Diagnostic measures for evaluation of thrombophilia

Summary

Using laboratory testing, coagulation alterations can be detected in about 50 % of familial thrombophilia. Most common hereditary coagulation defects leading to enhanced thrombosis risk are aPC resistance/Factor V Leiden mutation, protein C- and S-deficiency, prothrombin 20210A polymorphism and antithrombin deficiency. Moreover, elevated plasma levels of homocysteine also are associated with enhanced thrombosis risk. Severity of thromboembolic risk depends upon type of coagulation defect, hetero- or homocygosity and occurrence of additional acquired risk factors like immobilisation. Therapy of thromboembolic diseases must always be planned considering both clinical circumstances and laboratory findings.

Zusammenfassung

Mit Hilfe von Laboruntersuchungen ist es heute in ca. 50 % der Fälle möglich, bei familiärer Thrombosehäufung einen Gerinnungsdefekt zu diagnostizieren. Die häufigsten Gerinnungsdefekte, die mit erhöhtem Thromboserisiko einhergehen, sind die aPC-Resistenz bzw. Faktor V Leiden-Mutation, Protein C- und S-Mangel, Prothrombin 20210A Polymorphismus und Antithrombin-Mangel. Darüber hinaus werden erhöhte Homocysteinspiegel in Verbindung mit erhöhtem Thromboserisiko gesehen. Das Thromboserisiko des einzelnen Patienten hängt von Art, hetero- oder homozygoter Ausprägung des nachgewiesenen Defektes, sowie davon ab, ob zusätzlich erworbene Risikofaktoren wie z. B. Immobilisation hinzukommen. Die Behandlung thromboembolischer Erkrankungen muss sich sowohl auf die Laborergebnisse als auch auf die klinischen Befunde stützen.
