Granuloma trichophyticum Majocchi

Zusammenfassung

Wir berichten über einen 68jährigen Mann (R. D.), der 2 Monate vor Erstvorstellung erythematöse, juckende Papeln am linken Unterarm bemerkte, die zu einem erhabenen, erythematösen Plaque konfluierten. Klinisches Bild und histopathologische Untersuchung ergaben zusammen mit dem kulturellen Nachweis von Trichophyton rubrum ein Granuloma trichophyticum Majocchi, eine chronische Dermatophytose, die wie die Trichomycosis granulomatosa nodularis cruris und die Trichophytia profunda (Kerion Celsi) zu den tiefen Trichophytien zählt. Die orale Therapie mit Terbinafin 250 mg täglich über 2 Monate führte zur Abheilung. Hier soll anhand von klinischem und histopathologischem Bildmaterial auf diese relativ selten diagnostizierte Hauterkrankung aufmerksam gemacht werden.

Granuloma Trichophyticum Majocchi - Successful Treatment with Terbinafine - A Case Report

We report on a 68-year-old man (R. D.) who noticed erythematous, itching papules on the left forearm which developed to an erythematous plaque since 2 months ago. Clinical aspect and histopathologic examination together with mycologic proof of trichophyton rubrum led to the diagnosis of granuloma trichophyticum Majocchi, a rare dermatophytosis which belongs to the group of deep trichophytias like trichomycosis granulomatosa nodularis cruris and trichophytia profunda (kerion celsi). Therapy with oral terbinafine over 2 months was successful. With this case-report the authors want to call attention to this rarely diagnosed disorder supported by clinical and histopathologic photographs.
