Zusammenfassung.
Ein 72-jähriger Raucher wurde zur Abklärung von seit 10 Monaten bestehenden figurierten
Hauterythemen stationär aufgenommen. In der Anamnese wurde ein Gewichtsverlust von
5 kg innerhalb der letzten 4 Monate angegeben. Bei gutem Allgemeinzustand imponierten
an Stamm und Extremitäten anuläre, teils konfluierende und polyzyklische Erytheme
mit zentraler Abheilung. Eine Hautbiopsie zeigte die typische Histopathologie eines
Erythema anulare centrifugum. Als Zufallsbefund fand sich im rechten Lungenunterlappen
ein nicht-kleinzelliges Bronchialkarzinom im Tumorstadium III B. Nach einer fraktionierten
Radiatio verkleinerte sich der Bronchialtumor, und die Erytheme bildeten sich zurück.
Vier Monate später traten im Rahmen einer Tumorprogression erneut figurierte Erytheme
auf. Eine palliative Chemotherapie mit Cis-Platin und Etoposid führte zu einer deutlichen
Rückbildung des Tumors und zur Abheilung der Hautläsionen. Begleitet von einem weiteren
Rezidiv des Erythema anulare centrifugum zeigte sich 13 Monate später eine ausgeprägte
Tumorprogression mit Hirnmetastasierung und eine akute Beinischämie, an deren Folge
der Patient verstarb. Das Erythema anulare centrifugum kann als kutane Paraneoplasie
zur Erst- und Frühdiagnose sowie zur Verlaufsbeurteilung einer malignen Grunderkrankung
beitragen.
Erythema anulare centrifugum as leading symptom of a non-small cell lung cancer.
A 72-year old male smoker was referred to our clinic due to figurate erythemas lasting
since 10 months. The patient was in good general condition, except for a body weight
loss of 5 kilo during the last 4 months. There were annular and polycyclic, partially
confluent erythemas with central clearing on the trunk and extremities. A skin biopsy
showed the typical histopathological features of erythema annulare centrifugum. Further
examinations disclosed a non-small cell lung cancer in stage IIIB (UICC). After treatment
with fractionated irradiation, the tumour size decreased and the skin leasions disappeared.
Four month later tumour progression was accompainied by reappearance of figurate erythemas.
Chemotherapy with cis-platinum and etoposide resulted in both significant reduction
of the tumour and healing of the skin leasions. However, 13 months later, the patient
suffered from marked and finally fatal tumour progression with brain metastases that
was again associated with the development of erythemas. In conclusion, the erythema
annulare centrifugum belongs to the facultative paraneoplastic syndromes of the skin,
and can lead not only to the diagnosis of an internal malignancy, but may also help
to monitor the course of the disease.
