This video demonstrates near-total resection of a calcified craniopharyngioma in an obese patient via an expanded endoscopic endonasal approach with stalk preservation. A thin adherent residual was left to prevent hypothalamic injury. Postoperatively, the patient required hydrocortisone but had preserved posterior pituitary and visual function. With a stable weight he had no symptoms of hypothalamic injury. This video illustrates the feasibility of stalk preservation, hypothalamic injury avoidance and perforator preservation in challenging calcified cases.
