A 67-year-old male presented with acute left-sided vision loss, headache, and complete CN III and VI palsy, with a sellar mass extending to the left cavernous sinus and suprasellar chiasmatic compression. An endonasal trans-cavernous approach was used for tumor removal from the superior and inferior compartments, achieving gross total resection. The patient was discharged on POD-2, showing complete improvement of the CN III and VI function with a peripheral visual field exam at 1-year follow-up.
