This video showcases the extracapsular resection technique for a giant pituitary tumor in a 53-year-old obese male presenting with progressive visual impairment and bitemporal hemianopia. MRI revealed a >4-cm pituitary tumor without cavernous sinus invasion. Despite being traditionally used for microadenomas, this method is feasible and effective for giant adenomas, providing an avenue for gross total resection while preserving critical structures, including the pituitary gland and surrounding neurovascular structures.
