Pseudoaneurysms after penetrating traumatic brain injuries are challenging clinical entities to manage, particularly when they involve major vasculature. We present the case of a 14-year-old boy who suffered a penetrating TBI from a lawn aerator, after initial debridement, 7 days later he developed a M1 pseudoaneurysm. We successfully treated this with a M2-M2 and STA-M1 bypass with obliteration of the pseudoaneurysm. At the 1 year appointment postoperatively, he had returned to full ADLs.
