Vascular compression of the optic nerve via the internal carotid artery is a rare clinical entity. There are currently no case reports that describe a macrovascular sling transposition technique for this pathology. We present the case of 69-year-old woman with severe left vision loss and optic nerve atrophy with an ectatic left ICA. Utilizing a sling transposition technique, we were able to effectively decompress the optic nerve and prevent further loss of vision.
