Lateral Supraorbital Translamina Terminalis Approach for Resection of a Chiasmatic Cavernoma

Cavernomas of optic pathway are rare. This is a case of an 18-year-old female with visual decline and left homonymous hemianopia. MRI demonstrated a 2-cm chiasmatic cavernoma eccentric to the right. A contralateral (left) lateral supra-orbital (LSO) translamina terminalis approach was used to remove the cavernoma. At 4 weeks follow-up, the patient returned to her normal daily activities. Her visual acuity was almost normal, but the left homonymous hemianopia was persistent.
