Retrosigmoid Transtentorial Approach with Suprameatal Drilling (Kawase from below) for Gross Total Resection of a Large Petroclival Meningioma

This is a case of a 54-year-old male with headaches who was found to have a large left petroclival meningioma mostly locating in the posterior fossa but extending to the Meckel's cave and supratentorial space. A left retrosigmoid transtentorial approach combined with drilling of the suprameatal bone (reverse Kawase) was performed. Post-op MRI confirmed gross total resection. Patient was discharged home without any neurological deficits.
