We share our experience of a large ventral cervico-medullary meningioma resection in a 72-year-old female presenting with progressive myelopathic symptoms. The patient underwent a right-sided far lateral approach with C1–3 laminectomies in the prone position. In the video, we discuss the technical nuances of tumor dissection from the brainstem and spinal cord, highlighting cord rotation to maximize the lateral corridors. Gross total resection was achieved, with the patient showing progressively improving symptoms at 3-month follow-up.
