EC-IC/IC-IC Bypass with Radial and Descending Lateral Circumflex Femoral Artery Interposition Grafts for Giant MCA Aneurysm

A male in his 40s presented with an incidental giant right middle cerebral artery aneurysm with both M2 branches originating from the aneurysm itself. He underwent a combined EC-IC and IC-IC bypass using radial artery and descending branch of the lateral circumflex femoral artery interposition grafts to reconstruct the MCA circulation. The aneurysm was distally trapped with clip occlusion. Postoperative imaging confirmed bypass patency and progressive aneurysm thrombosis.
