This is an operative video of an endoscopic endonasal transpterygoid approach to the left cavernous sinus, middle fossa for resection of a trigeminal schwannoma. The patient is a 40-year-old male who presented with headache, blurry vision, diplopia, dizziness, nausea, and left facial pain found to have left CN VI and partial CN III palsy with imaging demonstrating a left cavernous sinus mass. We review his preoperative, operative, and postoperative courses.
