Endoscopic Decompression of a Large Left Petrous Apex Cholesterol Granuloma via a Transnasal Transsphenoidal Approach

This video submission details the endoscopic, endonasal resection of a left-sided petrous apex cholesterol granuloma in a 58-year-old male via transsphenoidal and transpterygoid approaches. Successful decompression and evacuation are demonstrated, along with the use of a right-sided nasoseptal flap at the cyst opening and cavity, left nasoseptal reverse-flap at the right nasoseptal harvest site, silastic placement within the cavity, and other key surgical highlights. The patient experienced significant symptom relief and is healing well postoperatively.
