Medial Transsphenoidal Endoscopic Endonasal Approach to Petrous Apex Cholesterol Granuloma

Petrous apex cholesterol granuloma (PACG) is an uncommon, cystic skull base lesion. Although benign, it can expand and cause local mass effect resulting to hearing loss, vertigo, facial palsy, and/or diplopia. We present the case of a 23-year-old male with bilateral PACG. The larger left PACG was marsupialized via medial transsphenoidal endoscopic endonasal approach while still clinically silent. A wide drainage to the sphenoid sinus with free mucosal graft was created to prevent recurrence.
