Direct Electromyographic Monitoring of the Lateral Rectus Muscle during Frontotemporal-Orbitozygomatic Approach for Resection of a Superior Orbital Fissure Schwannoma

The resection of orbital schwannomas is challenged by their proximity to oculomotor cranial nerves. Here, we present a case in which needle electrodes were placed retro-orbitally for electromyographic monitoring of the lateral rectus muscles during a frontotemporal-orbitozygomatic approach for the resection of a large superior orbital fissure schwannoma in a 39-year-old woman presenting with right CN VI palsy and proptosis. Postoperatively, MRI showed near-total resection and her exam demonstrated resolution of the right-sided proptosis.
