Extreme Lateral Approach for Foramen Magnum Neurenteric Cyst

A 29-year-old female with a cystic mass at the cervicomedullary junction that led to progressive right-sided weakness and sensory changes. An extreme lateral approach was performed with hemilaminectomy of C1 to access to the displaced spinal cord, enabling identification and transection of C2 posterior roots and the dentate ligament obtaining complete cyst removal. The patient recovered well from surgery without any complications.
