Endoscopic Endonasal Transpterygoid Transcavernous Resection of an Upper Clival Chondrosarcoma
A chondrosarcoma in the upper clivus and petroclival synchondrosis is treated with an interdural pituitary transposition and posterior clinoidectomy. This involves opening the anterior cavernous sinus wall, incising the caroticoclinoidal ligament, and ligating the inferior hypophyseal artery. Using a transpterygoid approach, the entire cavernous ICA up to foramen lacerum is exposed, guided by the vidian nerve and pterygosphenoidal fissure. This allows for removal of involved bone till the petrous apex and radical tumor resection.
