Trans-Sylvian Subtemporal Approach for Microsurgical Clip Ligation of Separate Basilar Apex and Posterior Cerebral Artery Aneurysms
A 46-year-old female with history of severe headache was found to have a basilar apex and a fusiform left posterior cerebral artery (PCA) aneurysm. The patient underwent a trans-sylvian subtemporal approach for microsurgical clip ligation. Technical considerations for using multiple operative corridors for clipping multiple posterior circulation aneurysms is demonstrated. Complete obliteration of the aneurysms was observed on postoperative angiography. The patient was discharged home with an oculomotor palsy that resolved at 1 month.
