A 68-year-old female presented with thunderclap headache and was found to have a ruptured right superior M2 dissecting aneurysm. The aneurysm re-ruptured within 2 hours of arrival leading to further neurological decline. She underwent side-to-side anastomosis with aneurysm trapping, which is presented in this video.
