Posterior Interhemispheric Approach for Resection of Falcotentorial Meningioma

Falcotentorial meningiomas pose operative challenge given their deep location and intimate relationship to deep veins. We present a 60-year-old woman who presented with progressive confusion, imbalance, and right-sided weakness. MRI demonstrated a large falcotentorial meningioma compressing the tectum and internal cerebral veins downwards. We performed a posterior interhemispheric approach for early devascularization, followed by gross total resection and excision of the involved falx and tentorial incisura, with preservation of critical neurovascular structures.
