We present a case of a spheno-orbital meningioma with superior orbital fissure, cavernous sinus, and anterior clinoid involvement. This case was approached via an endoscopic transorbital approach (ETOA) with anterior clinoidectomy and supports the safety and feasibility of this approach for the treatment of such lesions.
