Endoscopic Ultrasound a Diagnostic Tool in Idiopathic Acute Pancreatitis
Objective: This study was planned to determine the utility of endoscopic ultrasound (EUS) in patients with unexplained acute pancreatitis, and subsequent need of endoscopic retrograde cholangiopancreatography (ERCP) .
Methods: Patients with idiopathic acute pancreatitis were subjected to EUS. EUS diagnosis was compared with the final diagnosis and outcome in natural history.
Results: EUS revealed a cause of pancreatitis in 29 of the 51 subjects (57%), including microlithiasis in 10 (18%), chronic pancreatitis in 19 (40%), pancreas divisum in 3 (6.1%), pancreatic cancer in 2 (3.5%), and was not diagnostic in 10 (32%). During a mean follow-up period of 12 months, diagnosis changed in seven subjects (11%), and 11 subjects (31%) had ERCP because of recurrent pancreatitis or persistent pain.
Conclusion: EUS picked etiology in more than half of patients with idiopathic acute pancreatitis. Most patients did not require ERCP during the follow-up period. EUS is a useful modality in patients with unexplained recurrent acute pancreatitis.
