Zusammenfassung

Infektiöse Entzündungen der Hornhaut (mikrobielle Keratitiden) stellen potenziell visusbedrohende Erkrankungen dar. Je nach Schweregrad und Verlauf können sie bis zum Verlust des Auges führen. Entscheidend für das Sehvermögen ist, dass die Erreger schnell erkannt und zielgerichtet therapiert werden. Dieser Beitrag behandelt zunächst generelle klinische, diagnostische und therapeutische Aspekte mikrobieller Keratitiden und geht anschließend auf die Besonderheiten bakterieller Keratitiden ein. In einem zweiten Teil werden klinische, methodische und therapeutische Besonderheiten von Keratomykosen und Akanthamöbenkeratitiden besprochen [1].

Abstract

Infectious inflammation of the cornea (microbial keratitis) represents a potentially vision threatening disease. Depending on the infectious agent and the course of the disease it can result in a complete loss of the involved eye. Early diagnosis and accurate treatment are mandatory to maintain a sufficient visual acuity. The purpose of this review is to highlight and discuss characteristic clinical features, diagnostic procedures, and therapeutic aspects of three major forms of microbial keratitis (bacteria, fungi, and acanthamoeba; the latter two in the 2nd part of this article).

  • Bakterien, Pilze und Akanthamöben stellen die häufigsten Ursachen mikrobieller Keratitiden dar.

  • Die im Rahmen einer mikrobiellen Keratitis auftretenden klinischen Symptome sind bis auf wenige Ausnahmen überwiegend unspezifisch und erlauben nur selten Rückschlüsse auf das zugrunde liegende Pathogen.

  • Das diagnostische Vorgehen bei mikrobiellen Keratitiden ist säulenartig aufgebaut und umfasst primär die Kultur sowie lichtmikroskopische und molekularbiologische Techniken.

  • In-vivo-Konfokalmikroskopie (IVCM) und Vorderabschnitts-optische Kohärenztomografie (AS-OCT) sollten im Rahmen der differenzialdiagnostischen Abklärung von mikrobiellen Keratitiden nie als ausschließliches Diagnostikum angesehen werden, sondern immer nur ergänzend zu klassischen Nachweisverfahren wie z. B. der Kultur eingesetzt werden.

  • Eine immunsuppressive Therapie sollte immer erst nach Einleitung einer antimikrobiellen Medikation begonnen und vor Beendigung dieser langsam ausgeschlichen werden.

  • Patienten sind bei intrakameralen, intravitrealen und intrastromalen Gaben von Antibiotika stets auf deren Off-Label-Gebrauch hinzuweisen.

  • Ein chirurgisches Vorgehen im Sinne einer lamellierenden oder perforierenden Keratoplastik sollte nach Möglichkeit immer erst nach Abklingen der akuten Entzündung erfolgen, da die Prognose für das Transplantat, auch im Hinblick auf eine potenzielle Rezidivgefahr, ansonsten deutlich reduziert ist.

  • Die visuelle Prognose einer mikrobiellen Keratitis hängt entscheidend ab

    • von der Virulenz des Erregers,

    • vom Zeitpunkt der Diagnosestellung und Therapieeinleitung und

    • vom Ausmaß der Entzündungsreaktion bei Erstvorstellung.
