Round Punched-Out Ulcer Treated with Kali bichromicum

Skin ulcers present a significant challenge to the individual and the healthcare system. They are often treated at homoeopathic clinics at primary health centres but are seldom reported. A 68-year-old female patient presented with deep ulcer on medial side of right thigh for the past 3 months ([Fig. 1A and B]). Mild itching was reported. Ulcer started as a small papule. No other complaints, no aggravating and ameliorating factors were found. There was no past history of similar illness. Ulcer had well-defined edges. It was round in shape with black crust in centre. There was no significant discharge of any kind seen or reported by patient. Ulcer was surrounded by erythema and had oedematous appearance. The medicine was selected on the basis characteristics of ulcers with the help of following rubrics (Synthesis 2.0).

  • Skin, ulcer, deep

  • Skin, ulcer, crusty, black scab

  • Skin ulcers, dry

  • Skin, ulcer, round ulcers

Kali bichromicum 30 C/4 pills twice a day was given for a week. There was improvement after 1 week. Kali bichromicum 30 C, twice a day, was continued for the next 7 days. After 15 days, ulcers started healing, granulation tissues seen, and ulcers reduced in size ([Fig. 2]). Kali bichromicum 30 C discontinued followed by placebo. Healing process continued in following weeks ([Figs. 3] and [4]). Complete disappearance of ulcer was seen within 6 weeks of treatment ([Fig. 5]). Patient reported after 2 months with completely healed ulcer ([Fig. 6]).
