Dilemmas in Management of Intracerebral Aneurysms Associated with Primary Brain Tumors

Introduction: The primary brain tumors associated with cerebral aneurysms are rare in neurosurgical practice and multiple issues should be considered parallel to the management of the associated tumors.

Method(s): We present four cases of intracranial aneurysm associated with brain tumors. Two cases with anaplastic oligodendroglioma grade II and III with anterior communicating artery aneurysm, one of them treated by Y stenting and coiling and the other one with primary coiling of the aneurysm. Another case with invasive pituitary adenoma with left internal carotid cavernous aneurysm presented with severe epistaxis treated with flow diverter and coiling. The fourth case is of an invasive ductal carcinoma.

Result(s): In one case post coiling and stenting developed intratumorally bleeding which was controlled conservatively. In the other cases endovascular management preceded tumor therapy and in one case no treatment was considered

Conclusion(s): The clinical management of coincidence of brain aneurysm and brain tumors should consider many issues like the nature of the aneurysm, history of radiotherapy, type of adjuvant chemotherapy.
