A Decade-Long Experience with the Web Device: Tips, Tricks, and Challenges

Introduction: Wide-necked intracranial aneurysms remain a challenge for physicians to treat endovascularly. The Woven Endobridge Device has helped physicians overcome this difficulty, however, not without constraints. There is a learning curve with the Web device, especially for the lesser experienced operators.

Method(s): Current evidence supporting the use of the Web device in ruptured and unruptured aneurysms is explored. The author reviews his 10-year experience with the Web to identify salient cases looking at patient selection, technical nuances and complications.

Result(s): Short summary of the WEBCAST, French Observatory, WEBCAST 2, WEB-IT, and CLARYS studies is to be presented. The author then presents unusual cases illustrating indications for treatment with the Web device, strategies to aid device deployment in unfavorable anatomy, and management of complications.

Conclusion(s): The Web device has vastly changed the scope of treating wide-necked intracranial aneurysms enabling physicians to safely treat these challenging aneurysms endovascularly.
