Transfemoral versus Transradial Partial Splenic Artery Embolization in Patients with Hypersplenism

Introduction Purpose: To compare the transradial approach (TRA) and transfemoral approach (TFA) in patients undergoing partial splenic artery embolization for hypersplenism concerning the procedural variables, complications, and patients' opinions.

Method(s): A single-center prospective randomized controlled trial included 146 patients who underwent partial splenic artery embolization for hypersplenism. Patients were randomly allocated to receive transfemoral (73 patients) or transradial approach (73 patients). Primary outcomes included angiographic and procedural characteristics, access site complications, technical success, and patient preference, while secondary outcomes included long-term complications.

Result(s): Overall procedural success rate was 100% in both groups. Access failure reported in one patient (1.73%) in the TF group, Access site pain (3.66 ± 0.92 vs. 2.21 ± 0.54) and postoperative pain (3.98 ± 0.82 vs. 2.13 ± 0.62) showed statistically significant differences between TF and TR groups (p < 0.001).

Conclusion(s): TRA is a feasible and safe access route for abdominal and systemic interventions. It can be used as an efficient alternative to TFA with less complications, less pain, and higher patient preference.
