Hybrid Treatment of Iliac Segment Occlusion in Combination with Lesions of the Common Femoral Artery

Introduction: Hybrid treatment is a combination of endovascular and open surgery. This approach is optimal and appropriate for patients with occlusion of the iliac segment in combination with lesions of the common femoral artery (CFA).

Method(s): For the period from 2017 to 2021 hybrid revascularization of the ilio-femoral segment was performed in 142 patients with CLI grade 1 to 3 (Rutherford classification). In total, 122 patients were male and 20 patients were female. Of these, 83 patients suffered from grade I, 34 patients—grade II, and 25 patients—grade III. The choice of a hybrid strategy of surgical treatment is due to hemodynamically significant lesions of CFA in combination with ipsilateral occlusion of the iliac segment.

Result(s): In the perioperative period, no lethal cases were recorded. In patients with trophic changes, complete revascularization was performed, which led to the healing of ulcers in 100% of cases within 3 months of observation.

Conclusion(s): Hybrid interventions have become a generally accepted strategy for revascularization in patients with multilevel lesions. This approach allowed us to move away from the classic bypass surgery on the iliac segment.
