Background: There is no consensus on how to treat isolated iliac aneurysms. There is some benefit in limiting endovascular repair to iliac system to preserve blood flow from IMA and lumbar. We are presenting some cases in how to overcome sealing zone challenges.

Case: Case 1: reversed limb without extra-corporeal deployment of flared Gore limb. Case 2: cross-bifurcation access with option of limb extension using internal iliac Gore limb. Case 3: endovascular treatment of mycotic aneurysm using superficial femoral vein covered stent.

Discussion: Through various techniques, sealing of iliac aneurysm can be limited to iliac system with various techniques. With no increase on secondary interventions. We propose a new staging system which will help on planning those cases.
