Technically Challenging Balloon Dilatation of IVC and Hepatic Vein Using TIPS Set via Groin Approach

Background: An undescribed and nonconventional but successful case report of venoplasty involving a short segment of stenosed IVC and HV dilatation performed via a femoral approach. Case: a 45-year-old man with known Budd-Chiari syndrome had become refractory to medical therapy. CT liver dynamic was performed which showed patency of the right and middle hepatic veins. Whereas the hepatic venous confluence with the IVC and left hepatic vein where occluded and a short segment of the IVC severely stenosed resulting in collateral formation and congestive liver.

Discussion: Venoplasty/stenting of this segment comprising the outflow track of the right and middle hepatic veins was considered following review in the hepatobiliary MDT. Following initial access via the right internal jugular vein, the cavogram showed a severe short segment stenosis of the suprahepatic IVC.
