Introduction: This study aimed to evaluate the effect of interventional embolization on seizure presentation in patients with medium and high Spetzler-Martin (SM) grade brain arteriovenous malformations (AVMs).

Method(s): From the patients with AVMs treated with embolization those with medium and high SM grade AVMs suffering from seizures were included in the analysis. Postoperative follow-up was performed using a questionnaire and seizure presentation was assessed according to the International League Against Epilepsy (ILAE) and Engel classifications. Statistical analysis of patient characteristics association with ILAE and Engel scores was performed using Kruskal–Wallis and Fisher's exact tests.

Result(s): Out of the 61 patients, positive seizure dynamics (ILAE class 1, 2) were observed in 47.54%. A correlation was found between seizure type and outcomes of both Engel and ILAE scores, where those with focal onset had better outcomes and worse outcomes were found among patients with unclassified type.

Conclusion(s): Treatment of medium and high SM grade AVM patients with embolization had a positive effect on seizure and a relatively low prevalence of complications.
