Zusammenfassung

Hintergrund Ziel der vorliegenden Studie war es, Veränderungen von der Typ-2-CNV (klassische CNV) in der OCT-Angiografie (OCT-A) unter Anti-VEGF-Therapie aufzuzeigen und die Abgrenzbarkeit der Läsion vom umgebenden Gewebe in unterschiedlichen Segmentierungen zu untersuchen.

Methode Bei 12 Patienten wurde in der Fluoreszenzangiografie (FAG) und im SD-OCT eine gut abgrenzbare Typ-2-CNV mit assoziierten exsudativen Veränderungen im SD-OCT diagnostiziert. Bei allen Patienten wurde zudem eine OCT-A (Avanti, Optovue) vorgenommen. Diese Befunde wurden initial, nach den ersten 3 Injektionen und bei erneuter Läsionsaktivität bei einer Segmentierung auf dem Niveau der Choroidea, Choriokapillaris (CC), RPE und der äußeren Netzhaut verglichen. Bei allen Aufnahmen wurden die Fläche und der Flow ausgemessen sowie die Abgrenzbarkeit der CNV zum umgebenden Gewebe bewertet.

Ergebnisse Nach den ersten 3 Injektionen anti-VEGF waren eine signifikante Verkleinerung des Gefäßareals und eine Reduktion des Flow in allen 4 analysierten Segmentierungen zu sehen (p < 0,05). Am stärksten signifikant waren diese Veränderungen auf Höhe des RPE (p < 0,005). Bei Auftreten von erneuten Aktivitätszeichen im SD-OCT zeigte sich eine signifikante Vergrößerung des CNV-Areals und des Flow nur auf der Ebene der Choriokapillaris im OCT-A (p < 0,05). Bei Vorhandensein von Aktivitätszeichen in den SD-OCT-Aufnahmen ist die Abgrenzbarkeit der Typ-2-CNV vom umgebenden Gewebe deutlich besser, als wenn keine „Aktivität“ vorliegt. Besonders leicht fällt die Abgrenzung auf den Ebenen der Choriokapillaris und des RPE. Zudem zeigte sich im aktiven Stadium einer Typ-2-CNV ein vermehrtes Vorhandensein von Kapillaren, die sich nach Anti-VEGF-Therapie nicht mehr darstellen lassen.

Schlussfolgerungen Mittels OCT-A sind sowohl Größen- als auch Vaskularisationsveränderungen einer Typ-2-CNV detektierbar. Besonders signifikant waren diese Veränderungen auf Höhe des RPE und der CC. Auf diesen Ebenen war auch die Abgrenzbarkeit der Typ-2-CNV zum umgebenden Gewebe besonders gut. Die Ebenen scheinen sich somit für die Verlaufsbeobachtung unter Anti-VEGF-Therapie besonders zu eignen. Durch die Darstellung von Kapillaren bietet die OCT-A eine weitere Möglichkeit, Aktivitätskriterien zu beurteilen. Die OCT-A-Bildgebung dient als ergänzendes diagnostisches Verfahren zu den etablierten, dem SD-OCT und der FAG.

Abstract

Purpose OCT-A is a new method to visualise the 2D and 3D structures of neovascular complexes in exudative AMD. The aim of the present study was to characterise type 2 CNV in different 2D segmentations and in 3D imaging and to investigate changes during anti-VEGF treatment.

Methods 12 patients with type 2 CNV in FA and SD-OCT were selected. OCT-A (Avanti, Optovue) was obtained initially and after the first three injections and thereafter, if “new activity” (increase in sub- or intraretinal fluid) occurred. The characteristics of the type 2 CNV were classified initially and during follow-up in different segmentations (outer retina, RPE, CC, choroidea), in respect to the size of the CNV, the flow area within the CNV and flow index (% of flow area within the total lesion).

Results Comparison of the vessel characteristics before and after anti-VEGF treatment showed a significant reduction in the size of CNV at every level (p < 0.05). This was most significant at the RPE level (p < 0.005). After new activity, a significant increase in size was only recognised at the CC level (p < 0.05). Similarly, the most significant changes in the flow area were measured at the RPE level before and after treatment (p < 0.01) and at the CC level after new activity (p < 0.05). Demarcation from type 2 CNV of the bordering tissue was much better when activity occurred.

Conclusions OCT-A provides a new opportunity for the assessment of vascular characteristics of type 2 CNV, and quantifies CNV size and vascularisation under anti-VEGF therapy. This may be used in further studies in combination with SD-OCT scans to describe characteristics of this type of CNV under treatment. OCT-A is an additional medical imaging procedure to SD-OCT and FA, but more experience is needed in distinguishing CNV in the active and non-active stages.
