Pankreastransplantation – aktueller Stand und Probleme

Die Pankreastransplantation wird in Deutschland immer seltener durchgeführt. Eine restriktive Akzeptanzpolitik der Transplantationszentren sowie der Mangel an geeigneten Spenderorganen führen zu einer deutlichen Zunahme der Wartezeit potenzieller Empfänger. Für die wenigen Zentren, die noch ein aktives Pankreastransplantationsprogramm vorhalten, gilt es, die guten Ergebnisse der letzten Jahre auch unter Verwendung von zunehmend marginalen Spenderorganen zu erreichen. Die Marginalität beruht vor allem auf dem höheren Alter der Organspender und ihrer Komorbidität.

Kernaussagen
  • Pankreastransplantationen sind anspruchsvolle Eingriffe.

  • Sie werden lediglich an wenigen Zentren in nennenswertem Umfang durchgeführt und erreichen im Vergleich zu den sonstigen Organtransplantationen nur geringe absolute Zahlen.

  • Trotzdem stellen sie für ausgewählte Patienten eine wertvolle Therapieoption dar und können bei richtiger Indikationsstellung die Lebenszeit der Betroffenen verlängern und deren Lebensqualität spürbar verbessern.

  • Der Mangel an Spenderorganen und eine restriktive Akzeptanzpolitik der Transplantationszentren führen zu einem Anstieg der Wartezeit in Deutschland.

  • Fehler bei der Pankreasentnahme und Einschätzung der Organqualität treten im Vergleich mit anderen viszeralen Organentnahmen häufiger auf.

  • Die Anforderungen an pankreasentnehmende Chirurgen wurden neu formuliert.

  • Auch unter Verwendung zunehmend marginaler Spenderorgane werden in Deutschland international vergleichbare Ergebnisse nach Pankreastransplantation erreicht.
