The Emergency Echocardiography Simulation Test: A new assessment tool for echocardiographic findings related to peri-resuscitation care

Aim Identification and treatment of potentially reversible conditions in cardiac arrest is crucial and has to be completed within narrow time frames. Transthoracic echocardiography (TTE) may serve as an aid for this purpose, but requires basic skills for quick visual perception.

We developed the EmEchoSimTest™, which is a computer-based training program for the recognition of common peri-resuscitation echocardiographic findings. The feasibility of very rapid visual finding or diagnosis and its potential achievement within limited focused echocardiography training programs were evaluated

Methods The test is an automated program randomly showing sixteen test blocks. Each one displays echocardiography video clips for 5 (short version) or 60 seconds (long version) without replay, and requires diagnosis with a free text and to a multiple-choice question. Trainees underwent FEEL and FATE training as focused TTE schemes. Additionally, a group of TTE experts and one of untrained medical students were tested. Time to diagnosis and percentage of correct answers were main outcome variables.

Results 168 subjects were enrolled. Mean success rate for trainees ranged from 55,4% to 64,5%, and was 85,5% and 21,9% for experts and students, respectively. Trainee’s mean time to correct diagnosis was 22±13 or 33.6±6.2 seconds (mean±SD). FEEL course trainees identified several echocardiographic findings within 10 seconds, and PEA within 5 seconds. All the findings were recognized by experts within 10 seconds.

Conclusion As latest ERC and ILCOR (1) statement suggests training of Resuscitation Ultrasound the EmEchoSimTest™ is well suited to training of physicians in related sonogram patterns.[1]
