Endoscopic Optical Coherence Tomography for Determination of the Anatomy of the Tympanic Membrane in vivo

Introduction
For the evaluation of the diagnostic value of Optical Coherence Tomography (OCT), knowledge of the geometry of the healthy tympanic membrane is crucial. Literature data are mainly based on histological data and small sample sizes. Systematic studies in vivo are not available.
Material/methodology
In our study, 35 healthy ears of test subjects between the ages of 25 and 87 years (mean 46.7 years) were examined. We determined parameters such as longitudinal and transverse diameters, angles and the thickness of the eardrum at defined points in the central area of the pars tensa. The OCT system with an endoscope of 3.5 mm diameter uses a laser in the wavelength range around 1300 nm and a depth measuring range of 8 mm.
Results
The thickness of the eardrums, averaged over the defined points, was between 0.08 and 0.16 mm. For the individual points, the mean value of all eardrums was between 0.11 mm and 0.13 mm. The mean longitudinal diameter in the annulus plane was 8.7 mm, the transverse diameter 7.3 mm. No correlation between the diameter and thickness of the eardrum could be seen.
Conclusion
The examined eardrums showed a wide range in terms of their thickness. Overall, the results of our study are at the upper end of the range given in the literature. However, a shrinkage of the specimens of up to 30% due to the preparation for histology has been observed. With our results, reference data are now available for Optical Coherence Tomography of the middle ear.
