Cochlear implantation in children with congenital unilateral deafness

Introduction The aim of fitting children with congenital unilateral deafness with a cochlear implant (CI) is, similar to adults, to improve speech comprehension in quiet and in noise and spatial hearing by enabling binaural hearing. Thus, language acquisition should be as unimpaired as possible and, as a result, increased attention and concentration should be achieved for everyday (school) life.
Method Follow-up of 27 congenital unilateral deafened (SSD) children who have been fitted with a CI for at least one year. The aetiology of the hearing loss, the audiological outcome and the subjective assessment of the outcome by means of third-party assessment (CAP, SSQ) were considered.
Results The mean age at implantation was 25 months (SD 21 months), the mean duration of care at the time of data collection was 65 months (SD 29 months). In 10 (37 %) of the children considered, a malformation of the inner ear or the internal auditory canal was present, in 13 (48 %) children the aetiology remained unknown, in 4 (15 %) children a congenital CMV infection was assumed to be the causative factor. Overall, unilateral CI fitting is accepted by 23 (85 %) of the children with all-day wearing time (11 % non-user, 4 % limited use), in 20 children (74 %) localisation ability is detectable.
Conclusion Based on the results, cochlear implantation may be a treatment option for children with unilateral surdity of different aetiology. Important criteria for the decision for cochlear implantation seem to be the radiological assessment of the auditory nerve system and the duration of deafness.
