Cost-effectiveness-analysis of tonsillectomy

Introduction Rising costs and restricted resources in the financing of health care provision serve to highlight the importance of sound cost-effectiveness analyses. Cost-effectiveness analyses affect all subareas of medicine including otorhinolaryngology (ORL). Tonsillectomy is one of the most performed surgical procedures in ORL.
Methods Prospectively collected Patient Reported Outcome Measures (PROMs) data from 108 patients undergoing tonsillectomy at a German hospital were analyzed for the study. Study participants completed a generic PROMs questionnaire (SF-36) and a disease-specific questionnaire (TOI-14) before and 12 months after tonsillectomy. Health utility gains after tonsillectomy were combined with cost data to estimate procedural cost per Quality-adjusted life year (QALY) gained.
Results Patients who underwent a tonsillectomy had an increase in Health-related Quality of Life from 0.709 preoperatively to 0.826 one year later, resulting in a gain of 0.117 QALY in the first year. The average Incremental Cost Utility Ratio was 20,629.23€/QALY for a tonsillectomy.
Conclusion Economic evaluation evidence for otolaryngologic interventions remains limited. Our findings indicate that tonsillectomy is highly cost efficient, assuming the British NHS threshold of 23,500€. per QALY. The duration of the follow-up observation should be extended to gain more information about the long-term effects of certain interventions.
