Definitionen

Spricht man über die Vertikalrelation, so stehen zunächst einige Begriffe zur Klärung an, weil sich die Nomenklatur teilweise verändert hat [1], [2]: Vertikalrelation = Vertikaldimension = Untergesichtshöhe bei Einnehmen der statischen Okklusion oder in Ruhelage (–2 bis 4 mm).

Die Definition der statischen Okklusion – synonym mit maximaler Interkuspidation – wird durch die DGFDT (www.dgfdt.de/terminologie-nomenklatur) wie folgt festgelegt: statische Okklusion = Zahnkontakte ohne Bewegung des Unterkiefers in Interkuspidation.

Für die Ruhelage gilt folgende Festlegung (Ruheschwebe = alter Begriff): Ruhelage = unbewusste Abstandshaltung des Unterkiefers zum Oberkiefer bei aufrechter Kopf- und Körperhaltung.

Merke: Die Definition der Vertikalrelation setzt entweder die Einnahme der Ruhelage oder aber vollständig vorhandene Zahnreihen in Ober- und Unterkiefer voraus, die sich in maximaler Interkuspidation berühren (bei gesundem stomatognathem System).

Beschreibt man die Vertikaldimension, so muss man gleichzeitig angeben, welche der beiden voneinander unterschiedlichen Unterkieferlagen gemeint ist.

Ist der Kieferschluss durch größere Zahnlücken nicht mehr in der vertikalen Dimension abgestützt und gesichert, z. B. bei manchen teilbezahnten und natürlich allen unbezahnten Patienten, dann fehlen die Voraussetzungen zur Festlegung der Vertikalrelation über die Zahnreihen. Die Untergesichtshöhe muss in diesen Fällen über die Ruhelage definiert werden. Die Ruhelage hat bei vollbezahnten Menschen einen messbaren, aber variablen Abstand zur maximalen Interkuspidation.

Wird die Ruhelage eingenommen und ist der Patient vollbezahnt bzw. wird die Bezahnung über Prothesen wiederhergestellt, dann sind die antagonistischen Zähne nicht miteinander in Kontakt, und es kann ein Spalt zwischen den Zahnreihen von Ober- und Unterkiefer festgestellt werden: Interokklusalabstand = Abstand der Seitenzähne von Ober- und Unterkiefer bei Einnehmen der Ruhelage bzw. der Bestimmung des engsten Sprechabstands.

Für den engsten Sprechabstand gilt [2]: engster Sprechabstand = engster Abstand zwischen den Okklusalflächen bzw. Inzisalkanten der Ober- und Unterkieferzähne während des schnellen Sprechens.

Die Ruhelage steht im Zentrum der Begriffsbestimmungen für die vertikale Dimension, sobald der Unterkiefer nicht mehr durch Zähne abgestützt ist. Die „unbewusste Abstandshaltung des Unterkiefers zum Oberkiefer bei aufrechter Kopf und Körperhaltung“ gilt i. d. R. als „eingenommen“ oder „eingestellt“, wenn unsere Patienten einen „zwanglosen Lippenschluss“ aufweisen. Zwei Fragen stellen sich:

  1. Was heißt „zwangloser“ Lippenschluss?

  2. Wie ist es möglich, die intraorale Beziehung der Zähne oder der Kiefer zueinander, die man ja bei Einnahme der Ruhelage wegen der geschlossenen Lippen nicht direkt sehen kann, sichtbar zu machen?

Die „Untergesichtshöhe bei Ruhelage“ ist ohne Frage interpretierbar und in der Beurteilung sicher auch zum Teil von eigenen Präferenzen und der eigenen Erfahrung abhängig. Es ist daher i. d. R. schwierig, diese Position reproduzier- und verifizierbar einzustellen: Zwei unterschiedliche Zahnärzte werden nicht immer zu einem völlig übereinstimmenden Ergebnis kommen (s. u.). Für eine sicherere Beurteilung ist es wichtig, „das bewusste Hinsehen“ zu erlernen.

Definitionen [1],[2]

  • Vertikaldimension = Untergesichtshöhe bei Einnehmen der statischen Okklusion (engl.: occlusal vertical dimension; auch: vertical dimension of occlusion) oder in Ruhelage (–2 bis 4 mm; engl.: rest vertical dimension)

  • statische Okklusion (engl.: maximal intercuspal position; auch: maximal intercuspation) = Zahnkontakte ohne Bewegung des Unterkiefers in Interkuspidation

  • Ruhelage (engl.: physiologic rest position, auch: comfort zone) = unbewusste Abstandshaltung des Unterkiefers zum Oberkiefer bei aufrechter Kopf und Körperhaltung

  • Interokklusalabstand (engl.: interocclusal distance, interocclusal rest space, free-way space) = Abstand der Seitenzähne von Ober- und Unterkiefer bei Einnehmen der Ruhelage bzw. der Bestimmung des engsten Sprechabstands

  • engster Sprechabstand (engl.: closest speaking space) = engster Abstand zwischen den Okklusalflächen bzw. Inzisalkanten der Ober- und Unterkieferzähne während des schnellen Sprechens

  • zentrische Kondylenposition (ZKP) (engl.: centric relation) = kranioventrale, nicht seitenverschobene Position beider Kondylen bei physiologischer Kondylus-Diskus-Relation und physiologischer Belastung der beteiligten Gewebestrukturen

  • zentrische Okklusion (engl.: centric occlusion) = statische Okklusion in zentrischer Kondylenposition
