Otitis externa maligna and osteomyelitis of the temporal bone: examination of findings, course of disease and therapeutic measures

Introduction Otitis externa maligna/ osteomyelitis of the temporal bone is a rare and often threatening disease.

Methods From 2010 to 2020, a total of 39 cases of otitis externa maligna and osteomyelitis of the temporal bone have been diagnosed and treated. The cases were retrospectively evaluated with regard to microbiological and radiological findings, clinical appearance and therapeutic management.

Results The predominant symptoms at diagnosis were hearing loss, otorrhoea and severe otalgia. Pareses of the cranial nerves VI, VII, IX and XII as well as paresis of the recurrens nerve were often present. P. aeruginosa was most frequently (n=21, 54 % ) detected in the preoperative and intraoperative swabs. Radiological imaging frequently showed bone destructions. Common comorbidities were diabetes mellitus (n=26, 67 % ) and arterial hypertension (n=21, 54 % ). The vast majority of patients (n=37, 95 % ) underwent surgery. In all cases an antibiotic therapy was performed. In the follow-up after the end of therapy 10 patients (26 % ) were in good general condition and 23 patients (59 % ) in reduced general condition. 4 (10  % ) of the 39 patients died.

Conclusion Our study confirms the germ spectrum described in previous studies with the predomination of P. aeruginosa. The course of otitis externa maligna and osteomyelitis of the temporal bone is long and complicated. For the successful management of the disease extensive combined conservative and surgical procedures are required. However, even extensive therapies do not guarantee a favorable course.
