Notfallsituationen bei chronischen Hämodialysepatienten sind relativ häufig und eine belastende Herausforderung für Dialysepfleger, die meist als Ersthelfer tätig werden müssen. Ein geschultes, ruhiges und systematisches Vorgehen ist unabdingbar. Nach der Ansprache des Patienten und Überprüfung der Vitalfunktionen müssen Leitsymptome erfasst, Störungen des Bewusstseins, der Atmung und des Kreislaufs nacheinander erkannt und notwendige Erstmaßnahmen eingeleitet werden. In der vorliegenden Arbeit sind für die wichtigsten Notfälle Symptomatik, Erstmaßnahmen, Therapie und weitere Maßnahmen beschrieben. Zusätzlich werden Komplikationen der Hämodialysetherapie wie Hyperkaliämie, Blutdruckabfälle, Hämolyse und anaphylaktoide Reaktionen abgehandelt. Dieser Artikel soll dem Dialysepflegepersonal einen Leitfaden zum systematischen Vorgehen bei Notfallsituationen in der Dialysepraxis geben.
End-stage renal disease patients on chronic hemodialysis therapy often suffer from emergency situations which make heavy demands on the dialysis nurses in charge. Therefore, nurses have to be well-trained in order to act cool-headed and to follow a systematic approach. First, vital functions have to be checked, key symptoms of disturbances of consciousness, respiration and circulation have to be detected, followed by the initiation of first aid. This paper describes symptoms, first aid actions, acute therapy and follow-up actions of the most important emergencies. In addition, complications of the hemodialysis therapy such as hyperkalemia, intradialytic hypotension, hemolysis and anaphylactoid reactions are discussed. This compendium should help dialysis nurses to find a systematic approach to deal with emergency situations in the dialysis unit.
