Zusammenfassung

Die minimal-invasive, endoskopische Lungenvolumenreduktionstherapie hat die chirurgische Resektion fast vollständig ersetzt. Auch eine endobronchialeStenosetherapie und – in begrenztem Ausmaß – eine Tumortherapie sind heute interventionell möglich. Die verschiedenen Verfahren weisen jedoch verfahrensspezifische, anästhesiologische Besonderheiten und Risiken auf.

Abstract

Less invasive bronchoscopic techniques for lung-volume-reduction have almost replaced surgical resections. Tracheobronchial obstructions and – to a certain degree – even bronchial tumors can be treated bronchoscopically. However all these procedures show specific risks anesthesiologists have to consider.

Kernaussagen

  • Die endoskopische Lungenvolumenreduktionstherapie hat die chirurgische Resektionstherapie fast vollständig ersetzt.

  • Endoskopische Lungenvolumenreduktionen können entweder reversibel oder irreversibel durchgeführt werden, um beim Empyhsematiker die Lungenfunktion zu verbessern.

  • Eine endobronchiale kurative Tumorresektion mittels Laser-, Kryo- oder Argon-Plasma-Koagulation-Therapie (APC-Therapie) wird in den Leitlinien nur bei Carcinoma in situ der Schleimhaut empfohlen.

  • Endobronchiale Stenosen können interventionell bis auf wenige Ausnahmen nur palliativ behandelt werden.

  • Maligne endobronchiale Stenosen können entweder rein palliativ oder aber bis zur chirurgischen Resektion mittels Stentimplantation eröffnet werden.

  • Narbige Stenosen oder Verengungen durch extraluminale Raumforderungen können mittels Ballonkatheter rekanalisiert werden.

  • Bei Laser- oder APC-Interventionen muss zur Vermeidung von Bränden in den Atemwegen eine O2-Konzentration < 0,4 gewählt werden.

  • Eine Gas- / Luftembolie ist bei APC bzw. Laser-Interventionen eine lebensbedrohliche Komplikation.

  • Die Kryotherapie eignet sich auch für die Abtragung von Granulomen bei Metallstents.

  • Bei gemischt extra- und intraluminalen Stenosen können die verschiedenen Verfahren auch kombiniert werden.
