Neoplasm of the fallopian tube – peritoneal epithelial malignant mesothelioma

A 23-year-old woman presented with persisting lower abdominal pain and painful sexual intercourse for one month. Suspected of suffering from adnexitis, a treatment with Doxycyclin had previously been started. Diagnostics were performed and showed inconspicuously cervical smear tests, free liquid in the lower abdomen and an increased CA-125. In a subsequent diagnostic laparoscopy, a tubectomy was performed due to a swollen right fallopian tube.

The pathology revealed necrotic changes with mesothelial proliferations. The material was then sent to Vancouver for an expert opinion with the final pathological diagnosis being peritoneal epithelial malignant mesothelioma.

After finishing FertiProtelt, an explorative laparotomy was done. The malignant mesothelioma had spread to spleen, uterus, bladder, ovaries, the recto-uterine pouch and to the transition from uterus to vagina. A total parietal and partial visceral peritonectomy, splenectomy, ovariectomy and a resection of the sigmoid colon and the rectum was conducted.

7 days later, a HIPEC with doxorubicin and cisplatin followed. 3 months later, the patient presented for postoperative check-up. Due to new peritoneal abnormalities close to laparotomy’s scar, a re-laparotomy was performed with a the excision of all suspicious material. The pathological results however did not show any sign of malignancy. A MRI-control 4 months later didn´t show any malignancies at all.
