Objectives: Endovascular repair (TEVAR) of the aorta can be associated with significant complications.

Methods: We present a video of three cases requiring open surgical repair after previous endovascular repair.

Results: The first case shows descending aortic replacement using a custom-made pericardial tube for aortoesophageal fistula post TEVAR.

The second case shows total aortic replacement with a branched Dacron’s graft in aortic archaneurysm with severe type 1 endoleak post-TEVAR.

The third case shows replacement of distal aortic arch and descending aorta with homograft after coiling of aberrant right subclavian artery.

Conclusion: Although endovascular repair is nowadays commonly applied for aortic pathologies, severe complications can arise eventually requiring open surgical repair.
