Unclear paranasal bilateral tumors of the nasal root – a case report

In the area of the nasal root there are both, benign and malign tumors. While occurrences for infants and toddlers are primarily characterized by extracerebral gliomas or cephaloceles, skin tumors, which mainly occur unilaterally, are often observed on adults (e.g., pilomatricoma). Moreover, osteomata can develop on an inflammatory basis. In addition, granulomatous diseases such as Wegner's disease or sarcoidosis play an important role.

In our clinic we saw a 54-year-old patient who consulted us having bilateral tumors of the nasal root, which had existed for several weeks. The lesions were indolent, rough, spherical, palpable and well moveable below the skin level. An epiphora was denied by the patient. The patient was wearing glasses for many years, so the changes were perceived as severely disturbing. A CBCT of the paranasal sinuses and an MRI of the head could not reliably rule out contact with the nasolacrimal duct. A complete and unproblematic extirpation of the lesion was performed intraoperatively. Histologically, sarcoidosis could be detected. Further diagnosis and therapy was then carried out in a special clinic.

Based on the described case study, differential diagnoses of tumors of the nasal root are presented in this work.
