Clinical experience with self-expanding resorbable mometasone stents in functional endoscopic sinus surgery

Introduction:

Scarring and restenosis of the operation field are typical risks of endoscopic sinus surgery. Stenosis of the frontal or the ethmoid sinus can lead to headache or mucoceles, which can result in orbital or intracranial complications. Especially in the early postoperative phase, the local application of topical steroids in these critical areas is very difficult due to anatomical narrowness.

Methods:

Mometasone stents were inserted during functional endoscopic sinus surgery into the opening of the frontal or ethmoid sinus. We observed the clinical course as well as the occurrence of intra- and postoperative complications one week, 4 weeks and 12 weeks postoperatively.

Results:

We applied 15 stents in 10 patients with an average age of 55 years. The patients had an average of two previous sinus surgeries. Endoscopic application of the stent was feasible. The average duration for the application was 5 minutes. All stents were tolerated very well. There were no complications or dislocations. In no case, the stent had to be removed in advance.

Conclusions:

Easy application and the good compatibility of the mometasone stent make it a good instrument that can prevent early postoperative restenosis. Studies with higher numbers of patients and a longer follow-up are needed for better evaluation of cost effectiveness.
